Legal Statement & Terms and Conditions

1. ELIGIBILITY
The survey is open to legal residents of the United States who are 18 years of age or older at the time of entry, who are, either new or existing, customers of Performance Health and who submit a completed survey with their email address. Employees, officers, and directors of Performance Health, its subsidiaries, affiliates, advertising, promotion, and fulfillment agencies, and members of the immediate family or household of each are not eligible to participate. Performance Health customers who are Veterans Affairs, Acute Care, or purchase from Performance Health as part of a Group Purchasing Organization (GPO) are also ineligible to participate.

2. HOW TO ENTER
To enter, eligible participants complete the Performance Health survey and submit with their email address.

3. PRIZES
Prizes are non-transferable, non-exchangeable, and not redeemable for cash. The Sponsor reserves the right to substitute any prize of equal or greater value.

4. ODDS OF WINNING
The odds of winning depend on the number of eligible entries received.

5. CONDITIONS
By participating, entrants agree to abide by these Terms and Conditions and the decisions of the Sponsor, which are final and binding in all respects. The Sponsor reserves the right to disqualify any individual found to be tampering with the entry process or operation of the Sweepstakes or acting in violation of these terms or any other promotion.

6. RELEASE AND LIMITATION OF LIABILITY
By participating, entrants agree to release and hold harmless Performance Health, its parent, affiliates, subsidiaries, officers, directors, employees, and agents from any and all liability arising from their participation in the Sweepstakes or acceptance, possession, use, or misuse of any prize.

7. PRIVACY
Personal information collected from entrants is subject to the Sponsor’s privacy policy.

8. DISPUTES
Except where prohibited, entrant agrees that any and all disputes, claims, and causes of action arising out of, or connected with, the Sweepstakes shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate court located in the State of Illinois. All issues and questions concerning the construction, validity, interpretation, and enforceability of these Terms and Conditions, or the rights and obligations of the entrant and Sponsor, shall be governed by, and construed in accordance with, the laws of Illinois.

9. SPONSOR
The Sponsor of the Sweepstakes is Performance Health Supply LLC, 3333 Finley Road, Downers Grove, IL 60515.

10. NO PURCHASE NECESSARY to enter the sweepstakes.
Participants may enter by mailing their full name, address, phone number and email address to Performance Health, Survey Sweepstakes, 3333 Finley Road, Downers Grove, IL 60515. Each mailed entry must be postmarked during the sweepstakes period to be eligible, and only one entry per person is permitted unless otherwise stated. Void where prohibited.
