Product Recall  from DermaRite

This voluntary recall has been initiated due to certain individual lots of products having
microbial contamination that exceeds the acceptable limits set by the FDA. The FDA has
advised that the presence of elevated microbial levels may pose a risk of localized skin
irritation or infection due to elevated bacterial levels. Out of an abundance of caution we are
now expanding the recall to include all lots of the affected products and additional products.

Note: This recall should be carried out to the retail and user level.

Please review the instructions below and submit your response(s) to Performance Health.

Return Instructions

1. Immediately stop using the affected lots listed in the form table below and destroy all affected
   products in accordance with your facilities procedures. If destruction is not immediately possible
   at your facility, then the product should be quarantined until disposal. You may dispose of
   these lots in a standard trash receptacle; they should not be flushed or disposed of in septic
   systems or waterways.
